Added x-claude-code-prompt-id gateway hint headers for request grouping and observability

LLM gateways can now group requests belonging to a single user prompt using the x-claude-code-prompt-id header. Enabled via CLAUDE_CODE_GATEWAY_HINT_HEADERS environment variable to provide better visibility and routing control in gateway deployments.
