claude ultrareview command now requires explicit workspace trust

Starting with 2.1.309, the ultrareview command requires workspace trust before running. Terminal users receive a trust prompt; scripts must pass --trust-workspace or set CLAUDE_CODE_DISABLE_ULTRAREVIEW_TRUST_CHECK=1 to bypass. This is a security requirement for the cloud review feature.
