CodeRabbit PR Reviews adds author approval control setting

New reviews.allow_author_approval setting lets teams prevent pull request authors from approving their own changes using @coderabbitai commands, defaulting to true for backward compatibility. When disabled globally, the setting cannot be re-enabled per-branch, enabling stricter review separation policies.
