Supabase CLI v3.0.0 becomes the default install with local-first database commands and required confirmation for hosted changes

CLI v3 now runs database commands against the local stack by default instead of linked projects, requires explicit confirmation before any hosted changes, and unifies configuration precedence. This is a breaking change requiring CI pipelines and deployment scripts to add --remote flags and --yes confirmations.
