Plan Review Policy lets developers draft and review implementation plans before code generation

New /plan command enables developers to review and edit implementation strategies before code is written. Plans now appear as a separate agent mode. Rules budget expanded to 20,000 tokens, preventing large rulesets from crowding out skills and MCP tools.
